- Attach an itemized bill that identifies the provider or institution and address, Federal Tax Identification Number, NPI, patient, service or item, each service date, each charge, and diagnosis. Horizon claim form 7190
- Keep the total submission under 15 MB. Different visits to the same provider may be included in one submission when all claims are for the same member.
